Interoception, Trauma, and the Adaptive Body

The Role of Interoception in Trauma

Interoception is the ongoing perception of signals from within the body: heart rate, breath, muscle tension, gut sensations, and temperature changes. These signals form the foundation of emotion — not just the story we tell ourselves.


Emotion is a patterned interpretation of bodily change, shaped by past trauma, attachment, and learned survival strategies. Some parts of your system may amplify signals, others dampen them, and still others interpret sensations according to learned expectations.


This is why trauma, anxiety, and high-functioning stress are adaptive, not broken responses. The nervous system predicts danger based on past experiences, and these predictions shape what we consciously feel.


Why Attention Alone Isn’t Enough

Simply noticing your body isn’t enough for regulation. For many, unstructured attention can intensify protective activation, making embodiment feel destabilizing rather than grounding.

True regulated interoception requires:

  • Pacing — moving at a rate the nervous system can tolerate

  • Relational safety — internally (Self-to-part) and externally (therapist-to-client)

  • Permission from protective parts — letting each system component engage safely

  • Self-leadership — remaining present without being overtaken


Signal vs. Noise: Learning the Body’s Language

Some sensations feel like overwhelming “noise” to one part but meaningful “signal” to another. The goal is not more intensity but differentiation and relationship:

  • Which part experiences this as too much?

  • What does this sensation mean to that part?

  • Is there another part who interprets it differently?

Embodiment is relational, not mechanical. Regulation is co-regulated before it becomes self-regulated, which is why working with a trained practitioner is critical.
